Dear Foster Parents/Guardians/Caseworkers,

Following is a camp application for Teen Reach Adventure Camp (T.R.A.C). Please complete this application ASAP to reserve a space for your youth. Space is limited and we accept applications on a first come, first serve basis.

The cost per youth to attend camp is $425, however, grants are available.

If your child is currently a foster child/ward of the state, please complete a grant application from Arizona Friends of Foster Children (AFFCF) for $425. Applications can be completed online at: https://www.affcf.org/awards/. AFFCF requires supporting documentation. For your convenience, you can download a copy of this letter here to upload to AFFCF in order to meet this requirement.

The VENDOR name for the online grant application is “McDowell Mountain Community Church”.

If your child is not approved for a grant by AFFCF or is not a ward of the state, we have a limited number of scholarships that we can provide to youth.

PLEASE COMPLETE ALL INFORMATION ON THE APPLICATION, SIGN ALL WAIVERS AND UPLOAD REQUIRED DOCUMENTS.
